What is Support Coordination?
Support Coordination is a type of capacity-building support that is funded by the NDIS.
Many people may have complex needs, no informal support network or going through significant life changes. You may need help with understanding your plan and knowing what services or supports can be provided within your plan budget. Your coordinator of support helps you to link the right service providers of your choice to meet your needs.
Support Coordination can include individualised design and exploration of shared living and accommodation options.
What does a Support Coordinator do?
Support Coordinators will help a participant to:
- assess all the options for mainstream, community, informal and provider supports
- help to choose preferred options or providers and over time increase your capacity
- organise assessments that may be required to determine the nature and type of funding
- negotiate services and prices, develop service agreements and create service bookings
- increase your options to link with mainstream or community services
